For every KPI, document the formula, numerator, denominator, source, owner, refresh frequency and scope so departments do not calculate different answers.
Separate leading measures from outcome measures: the former should expose backlog or risk early, while the latter confirms delivery, cost or cash results.
The dashboard must drill down to source transactions and owners; review causes and actions regularly, retaining completion evidence and removing measures that no longer drive action.
